That for some time past and since the ........................day of.............or thereabout, the said C has become in same and is of unsound mind and can not manage himself/his properties and can not take care of his person. Two medical certificates giving his mental condition are unclosed here in.

5. That the selections prayed here below are compulsory for proper custody/care of person of C and managing of his estates.
